that's correct arlete!

i don't know the result yet. i took the cs at the end of april. i have 92 on step 1 and i want to take ck in august!
i worked hard for step 1 and after i got my score i said...."that's it, from now on is very easy"-hihihi big mistake!

but i still have faith and i pray the lord for help! smiling face

hi arlete....how is ur studying???

im pracising online but i still cant find a live partner,,,do u think it matters???? i dont think i can take my exam without practising live at least 2 days

dr naik: The awkward pauses tend to diminish with practice. Try and train yourself to be quiet during this pauses, not say "hmmm" or "OK", or fill it up any way. It's OK to have pauses to quickly organize your thoughts, but you don't want to sound insecure. When is your test?

iarumas: I still hope you'll pass.

paheli: I think First Aid is enough. If you prepare well, you'll be ready for any case that comes your way. Some questions are used for every case, no matter what the CC is, such as "when did it start, is it getting worse, better or the same, what causes, what alleviates, what makes it worse" and others, then the PAMHUGSFOSS. So even if they get creative (which I doubt), you'll know what to do. Yes, at least twice for all the FA cases. How many a day? I don't know... It depends on the number of children you have and if they have the chicken pox or not. wink Just kidding, but the more, the better. If you have Skype, give me your id, sweetie.

yara: Practicing online worked for me, although I had live partners, too. But to be very sincere, the PE part (the only thing we miss during online encounters) I didn't practice at all. Well, not untill the last day before the test, when a friend I met on the Internet (that had the test at the same time and day as me) went to meet me in the hotel, and we practiced around 8 cases, then we pretended to do the physical exam, but still very much pretend, so we were faster than in the real test. But if you've done PE in real patients before, and read the book, that will be simple. My study has been great, thank you.

I'm sorry, but right now I am studying for step 3 and as soon as I answer the phone, it's sure my kids start acting out! Skype is very convenient for me because when and if I have at least 20 minutes to dedicate to somebody without interruption, I make contact and am able to practice in timed mode.

Hello, Arlete: I really enjoy reading your posts here. I feel you are like my big sister. I have one Q about CS. I have problems writing notes when I take pt hx. I am an IMG. My english is considered OK by many people. But english is still my 2nd language. I feel I always get distracted when I write down the notes, and then forget what I need to ask for next questions. Thanks and GL for step 3.

What do you mean by "writing notes"? During the encounter, in front of the patient? If it is, you have to try and write the minimum possible, because you have to make eye contact the maximum you can. What I did was: before entering the room (but always after the announcement "this encounter starts now"), I wrote down PAMHUGSFOSS and LIQORAAA (if pain) and any word to remind me about things I usually forgot, such as "trauma", "fundoscopy", "thyroid symptoms", etc. Then, with the patient, each question I made I would put a check mark beside the letter, then jump to the other letter next. I wrote down only a few information, such as duration of symptoms, medications and dose, age of menarch, things I could forget. But during the test you are so aware of everything, you won't forget most of the things by the time you seat to write the PN, so leave the writing for the actual PN, not when interacting with the patient.

hi arlete..
i am asking too many history q's .. i mean i don't have the time judgement..
i rule out the three most imp d/d which i finish in time.. and then i am always tempted to ask more q's to rule out others .. thinking what if he has it????
i am now thinking of making one pertinent q for each d/d and then going into depths
what do you think?

The SPs lead us with answers that close pretty much to one single diagnosis, because the cases are very common and symptoms very typical. What I did was, once I had a "no" for answer, I went quickly to another question. If I had a "yes", I went on a little further for details. But if you get to the "5 minutes left" still making questions, drop everything and go wash your hands.

The ideal is not making questions during the PE or right after it, because you may sound disorganized or insecure. But if you have to (like I did), better doing them than missing those points. Once you start doing the closure, no more questions! You're supposed to know what you are talking about by that time.

kaplan audio lectures say tht u can ask anything about history at any point of time even while u are abt to leave the room
i think it is in lecture no.2 audio....
like they hav given an example when u forget to ask abt alcohol intake u can even ask b4r leaving

but as arlete said try to be more in a sequence

I hope there aren't many times when 5 minutes are left and i am still on q's, although i am in that situation many times now.

About P/E I used to prefer asking about say edema/knee swelling/ restriction of movements( like in knee pain) just before i examined it..or maybe history of tick bite.. so it still keeps the conversation up. or maybe before touching chest/abdomen ask for h/0 pain.

but i will try to be thorough in the beginning itself, to avoid sounding insecure , like you said..

Thanks ANGEL for the input, that tells me that better not make a blunder of not asking some pertinent history..I think such a situation will arise when we are running absolutely out of time.
