Wow , big congratulation , ECFMG certified , officially will b MD . GL in ur st 3 & residency , sure will ask u when my CS come , please ahng in here st 1, 2 , CS so we can reach you , thnx . Hats off for ur success with family , 3 children , job , exm all done for ECFMG . Such an inspiration u show lead us . What do u think about the book called kap CS 35 cases by Dr. Phil Brottman & any kap CS books ... Q book , Clinical Skills book ?? Do u practice all FA 31 cases & UW cases plus mini cases ? Which books u have to follow for FA, UW, kap ??? ......for Hx taking, in asking Qs to sp , writing sp notes from cc to Dx , DDx , Investigations and physical exm [ which one to follow, kap , uw video , etc ] in room , written part on sp note , plus councelling in the room ?? arlete....I envy your confidence.... I wish i can be as confident and relaxed as you were in your exam. I have it on 24th this month and still way way behind in memorizing things and in pracitce. Wish me luck!!!

Thank you, srita! shweHope: I don't know those books, sorry... I practiced all the big cases in the end of FA at least 3 times and read the minicases once. I only read uw, didn't practice those cases. I think FA is enough to follow and give it a personal touch when counseling. I bet you've done it a thousand times already. You may listen to the Kaplan audio if you have the chance to, and watch a few PE videos (there are some links around here). But the key is practicing, for sure. You get better and better from practicing, the questions start to come naturally to your mind and you stop having those awkward pauses you do at the beggining. oldkidontheblock: maybe I am delusional! Maybe it is my personality. I feel very comfortable around people, I talk easily to strangers and don't care a lot about other people's opinion about me or what I am doing. I care a lot about their feelings, though. So, after the first two cases, I pretty much was only being myself. Let me know if you have Skype, maybe I can help you get there. I don't have a lot of time, but I certainly have a lot of ideas...

Thanks for the offer. Skype is blocked here. Anyway, I have noted down your valuable piece of advice and believe me practising is helping me. I am doing FA and I agree that doing it three times would be the best thing to do. I may try to do those cases in UW which are little different from what in FA. My only concern at this time is writing PN. Can you give me some tips on it? One more thing after reading the doorway information, if I wait outside and write down my DDs and nemonics for the history, it would delay my entry into the room, would it affect if i enter after lets say 30-40 seconds ?

It didn't affect me...! I wrote "LIQORAAA trauma" for pain, PAMHUGSFOSS for every case and 3-5 pertinent questions (or a word that would remind me, such as weight, neuro, LOC) before entering.

Hi arlette How do we open a pediatric case.FA says that we have to address the pt.s name correctly.Like if the pt.s name is Josh white,18 month old......So if we have a phone case or a pt.s mother in the room.......we address her as mrs.white but is it necessary to mention the pt.s name also If yes what will we call him.....little jhonny?

I think I would prefer calling Josh if his name comes up in the conversation and address the mother as Ms. White Thanks arlete for the tip. I think writing down is the most sensible thing to do even if it costs us few seconds. I think the most important part in every case is History taking, counselling/closure. PE can be little relaxed as it technically not judged and PN can be lineintly marked as everyone has a different style, but I think SP marks every point in history. And yes manners do count.

Ask the mother's name. Sometimes (like in my case) it's not the same. Yes, you refer to the patient by his name, or "little name'. That's how I learnt, because I actually didn't have a pediatric case in my test. I agree with everything you said, oldkid. I put a little mark by the side of each letter I asked for, so I knew what would be the next question.

RX 135: only if your English is very good and you are used to seeing american patients and writing american patient notes every day. I would give it at least 3-4 weeks and try and spend most of the time practicing with somebody. sachin: I am not the best counselor about step 1, I got only 207/84...

What do we have to write in ROS? Can we write days as x/7 month as x/12, week as x/52 as its done normally in clinical practice in most part of the world or we do have to write the whole word all the time?

oldkid: I've never seen it written like that, they don't do it in Brazil... There's a list of abreviations allowed in the test in the usmle website. Anything that is not in that list will be used at the candidate's own risk, if you know what I mean. For ROS, I wrote just like in FA: "negative except as above". So I tried to write all the pertinent present and absent symptoms in the HPI, and tried to keep it in 5, maximum 6 lines.
