this is vikram.I am an i m g from india.i have read your post ,its very helpful.i have my xa at houston on 31 oct.i have read usmle world once.i practiced so far with an online friend and also with my brother .i think you might have tired by hearing to cases presented to you by aspiring doctors.but anyhow as a last ditch effort i want to present a case to you. i have skype id .i am in ca. tell me if it is possible for you to listen to a presentation from me.

dear arlete, this is srita iam retaking exam next month, when iam practicing iam running out of time in neuro cases so i want to ask you, in memory loss case during pe if i can do mmse and neuro exam first followed by cvs and pulmonary and other exams, so even if i run out of time atleast i will finish mmse and neuro exams, please answer, i will be wrong if i do that?

srita: I was never able to do a neuro case within 15 minutes when I was practicing. My plan was: if I reached the "last 5 minutes" warning and was not already doing PE, I would have done MMSE and a quick neuro exam, and go ahead and do the closure. I would sacrifice CV and PULM exams in order to talk to the patients about possible Dx, tests, counseling, "do you have any questions for me?". I think there are more points to gain during the closure.

But I didn't have any neuro case, so it's only my guess.

hi arlete, please tell me in neuro case to check sensation do we need to do pain temp dull and sharp senations vibration 2 point dicrimination all of that or is there any short cut , i mean can we omit few and do only some, please answer, because if wedo all that time will not be sufficient

Like I said before, srita, I didn't have a neuro case in my real test (lucky!), but I would have asked the patient to close his eyes, touch very lightly: 2 points on face, both arms, both legs, and ask: "Can you feel this? Can you feel this?" and that's all. And write on PN: "sensation to touch grossly intact".

Vibration on the legs and proprioception (big toe position) are important (and I did it only) for DM cases.

Hi Arlete. I'm new in this forum & I've been reading the posts. You seem to be the right person to ask coz you've been so helpful to everybody (with that beautiful smilesmiling face.

Anyways, I want to ask you when testing the motor part in a Neuro case, can you do the left & right simultaneously? Do I have to test each muscle group one at a time? I practiced with my husband yesterday & 15 minutes is not enough especially when you're doing neuro exam.

Yes, you can do them simultaneously. If the patient is faking a motor deficit, you'll notice the difference right away.

CNS cases are the most challenging in terms of time management, for sure! nod
