A patient with severe iron deficiency anemia refractory to oral ferrous sulfate therapy is given multiple blood transfusions in preparation for surgery. She develops hemosiderosis. Which of the following microscopic findings would be expected?

a.-Hepatocytes damaged by intracellular iron accumulation

b.-Intense lysosomal staining with methylene blue

c.-Storage of excess copper in hepatocytes

d.-Yellow- brown granules in cell cytoplasm

D.yellow brown granules..??

a.-Hepatocytes damaged by intracellular iron accumulation - in posttransfusional hemosiderosis usually is not seen damage as in Hemochromatosis

b.-Intense lysosomal staining with methylene blue - Prussian blue is for iron

c.-Storage of excess copper in hepatocytes- obviously not

d.-Yellow- brown granules in cell cytoplasm - that looks like hemosiderin granules to me.

So D is answer.

Hemosiderosis is due to increased total body iron content and is associated with intracellular storage of excess iron as ferritin and hemosiderin. Iron may be found not only in the organs in which it is usually found but also in the pancreas,heart,kidneys, endocrine organs, and skin. This iron excess may be due to high levels of dietary iron or blood transfusion. Hemosiderin, a denatured form of ferritin, appears microscopically as yellow-brown granules in cytoplasm.

Accumalation of iron( hemosiderin) mostly in in macrophages as yellow granules define hemosiderosis. While in hemochromatosis the accumulation is in the tissues and hepatocytes contain blue iron granules with Prussian blue.

yes in Hemosiderosis due to excessive iron load there is a deposition of degraded ferritin called hemosidrin which appears as yellow brown granules in the cytoplasm...nodnod
