Simple the lady clear shows signs of distress and not depression. Her stress is making her more lean towards depression. therefore will have some increased levels of Cortisol. Cortisol will decrease her ability to mount a effective immune response.

Some members go for Choice D as their answer but i dont why they choose it. Cortisol is the major stress hormone and Cortisol itself decreases TSH levels. The lady in question is having an Adjustment disorder. Anxiety or depression do tend to occur with HyperTH but they resolve with its Rx but they wouldnt show increased TSH.
