what all should be written on scrap paper before entering the exam room for cs?

if you memory is leaky, you can write whatever you want, but remember that this will take your time... I wrote only name and VS since I preferred to describe them on a PN rather than simply state WNL... During the encounter, write only positive symptoms and NKDA if the Pt has no allergies... Otherwise, you will forget if you asked about this or not...

I write "LIQORAAA, trauma" for pain (I always forget to ask about trauma), "PAMHUGSFOSS" and a few key words of questions I need to ask according to the case, for example "fever, weight loss, pruritus, thyroid symptoms). I write fundoscopy, mini-mental, complete neuro" sometimes. Even when it takes 1 minute for me to do it, the history is faster and more effective than if I don't (at least during my practice cases).
