@nnl I agree with answer for 1st question. 2.If baby is suffering from Albinism,according to kaplan notes,there is an increased risk of developing basal cell and squamous cell carcinomas.According to USMLEWORLD,there is an increased risk of developing skin cancers of all types. Sun exposure is a risk factor for basal cell carcinoma,squamous cell carcinoma,and malignant melanoma. Can you please explain your reason for picking squamous cell carcinoma? 3.Bronchial obstruction with a mucus plug,tumor,or a foreign body can lead to atelactasis. Fever is an important in diagnosing atelactasis,but question says that patient is afebrile.And atelactasis clinically causes signs of consolidation which includes increased tactile fremitus,but the question again says that there is decreased tactile fremitus in the patient. Is there something in the question iam missing?

My answers were: 1.Multinucleated giant cells 2.Squamous cell carcinoma....i dont know the reason but i just picked that choice....out of 3 skin cancers i picked this one by lottery 3.Pleural effusion.

About squamous cell i just choose Squamous cell and i didnt look it up yet but i had read somewhere that there was a co-relation and so choose havent looked it up yet. I choose bronchial obstruction cos Decreased breath sounds and tactile fremitus with TRACHEA SHIFTED TO THE AFFECTED is the biggest clue to Bronchial obstruction. I was going with pleural effusion first and then saw trachea shifted to right went back and re checked the affected side. and then choose Bronchial obstruction. Cos both have the same dec sounds and percussion and tactile fremitus while the only difference is that trachea shift decides whether its effusion or obstruction. Only in cases of resorption atelectasis would you see the opposite shift otherwise its same sided.

It took me a couple of classical patients and then our Prof asked us to look very closely at the trachea and we will see the shift. And then helps in making a faster diagnosis then reconfirm it with a CXR but you have an idea of whats going on.
