my take - University IM position - more prestigous, opportunities for great research, great opportunity for fellowships after residency community IM position - usually more autonomy and alot of hands on, fellowship opportunities after residency are slim (especially in the cardio and gastro),

University: -more research opportunity -easier fellowship acquisition, especially if program has in-house fellowships. -more prestige, but not in the sense that everyone thinks. Trust me, no one....in 20 years will care whether you did your residency in a community program or university program. In terms of job implications, there are PLENTY avaiable for all. So, in essence........the only advantage is FELLOWSHIPS. Community Program -less research oriented, more hand-on learning -sometimes less attention paid to teaching and more on getting the work done. -less fellowship opportunity Basically, if you want to specialize....Id go for a Univ. program. You'll definitely increase your chances, plus you'll get the research opportunity to imrove your resume. If you want more hands-on experience, smaller program with less residents, and more "intimate" relationship with attendings, Id go for community program.
