I failed twice, ones on ICE, because didn't take this exam seriously, but the second time was high on that, but failed on SEP, was depressed, hopeless, didn;t know what to do,how to improve my english. So, again practiced with partner who's english is native and asked to correct my q. It helped. and the third time was very afraid to talk at all but just TRY AGAIN!!! JUST AGAIN AND AGAIN, AND YOU WILL PASS!!! I know it...

Sorry to hear your stories. Wish you gather your courage and prepare for the next battle. Actually I don't think this test is about your English level. It is all about routine stuff in a clinic. So memorizing all the questions you may ask during the encounter on FA, and try to memorizing all the answers of challenging questions on FA would help you a great deal if spoken English is your weak area. After you memorizing those basic stuff, you just need to adjust a little bit when you face the real situation. Just my two cents. Maybe is wrong since I still not appear for CS.
