Hi all, I hope you don't mind me asking the basic Qs here. I forgot lots of things from S1 and S2CK prep. 1. Why do we check LDH in the case of confusion? 2. Why is BUN increased in upper GI bleeding? -mee.

In GI bleeding, there could be at least two reasons for BUN increase: first, blood is digested in GI tract, and blood proteins are digested to aminoacids that are absorbed; final product of aminoacids metabolism is urea - therefore, blood urea is increased; second, if there is huge bleeding, haemorrhagic shock develops with compensatory mechanisms like peripheral vasoconstriction in order to provide more blood for the brain and the heart, however, kidneys and other organs get less blood and renal hypoperfusion leads to prerenal azotemia
