64 yr. old man is diagnosed with chronic bacterial prostatitis and urosepsis. He is treated with large doses of broad-spectrum antibiotics for an extended period of time. Which of the following complications would he be most likely to develop?

A. Bleeding in joints >> DOC in CBP is TMP-SMX and SE of it is thrombocytopenia


Edit: Initially i thought this wold be something about a dude who overdosed on Cialis or something and has some other related issues. nod

because intestinal bacterial flora is gone. so no vit K no coagulation factors 2,7,9,10. no gamma carboxylation reactions in the liver. Similar to warfarin overdose.
Rx. fresh frozen plasma or vit K or synthetic clottting factors.

Great question. Could be TMP-SMX or Quinolones. TMP-SMX can cause thrombocytopenia. Quinolones really cause tendon problems rather than bone. Think I'll go with choice A.

CORRECT ANSWER: A. BLEEDING IN JOINTS

To answer this question you have to identify 2 pieces of information. First, you have to recognize that the question is about vitamin deficiency acquired by antiobiotic therapy (Vit. K is made by bacteria in the gut) and then you must recognize the deficiency syndrome that would be produced (bleeding tendency secondary to the inability to make clotting factors II, VII, IX, X, and proteins C & S).

Vit. D deficiency can lead to bony abnormalities.

Vit. A deficiency can cause decreased night vision.

Vit. B12 and thiamine deficiency can lead to neurologic defects.

Vit. C deficiency can lead to scurvy.

Yeah answer is A....due to Vit.K.deficinecy secondary to destruction of normal gut flora by chronic use of antibiotics.
I think it cannot be due to thrombocytopenia....bcas platelet disorders will not cause bleeding in to joints,which is a feature of coagulation factor deficiency.Platelet disorders cause epistaxis,petechiae,easy bruising,etc.
