A 55 yo F with a PMH significant for a prior MI and HTN is suffering from unilateral throbbing headaches preceded by a 10 min visual aura. The headaches last between 10 and 24 hours and are accompanied by photophobia. She had tried NSAID drugs to stop the headaches once they occur, but they are not effective. Which medication is the next logical abortive therapy to try in this patient?

A Acetominophen
B B-Blockers
C Ergotamine
D Triptans
E Valproate

D- triptans.

triptanes and ergotamines are contraindicated in pts w/ CAD (Hx of MI!)
valproate and beta-receptorblockers can be used for prevention (beta-receptorblockers are preferred in this case),
but for acute migraine attack: acetaminophen

Triptans and ergotamine contraindicated

B-blockers and valproate are prophylactic treatment

The only abortive therapy left is acetaminophen
