male 45 yrs, h/o smoking and COPD since 30 yrs,
accidental diagnosis of solitary pulm nodule in left upper lobe.
feature suggestive more of malignancy,

next step,

excisional biopsy
CT chest
obsevation
pulm function test
bone scan

CT chest...i go with this coz u can confirm radiological features of malignancy plus delineate its extent.

excisional biopsy is a tempting close choice but generally the lesions r much farther inside thn the reach of the excisional biopsy needle... this is generally not the first step described acc to most sources.

so the histological diagnosis needed for diagnosis of cancer has to probably be provided by bronchoscopy with biopsy or excisional biopsy if its too peripheral...but this has to be preceded by CT is what i feel.

oopsy...in my post i've written bout the biopsy needle...sorry bout tht....realised now tht its EXCISIONAL biopsy...so there's no needle...sorry bout tht...but this makes CT a stronger choice coz excisional biopsy is a surgical procedure...so definitely CT before tht.
