Hi there, My friend had a big question about a case where the patient gives you a form to fill the physical examination findings for an insurance work up during the CS exam.The FA points out that in such cases the patient will inform you that history taking is not necessary. So, the question is , Do we need to do a PE and fill out the form within the alloted 15 min (or) Do we need to fill out the form during the allotted 10 min for the PN.

its just my idea. i think if patient ask you to fill up form for examination. well thats obvious we start with asking chief complaint...and proper histiry then we can examine...otherwise we dont know wht to examine... so, i think proper history and examination...and counseling is still needed for those 15 min... and.. then if pt give u some form probably thats inclueded as patient note.. then i guess examiners wont give us patient note.
