if patient says to the physician that he feels worthless, with insomnia, loss of appetite, sad mood since 2 months and feels that its not worth living anymore and sometimes feel that he shud give away his life.

what will u do??

- ask him whether he feel like doing suicide.
- start SSRI right now
- admit him and go for ECT

Ask him about suicidal ideas is a must

oh...thats why i got it all wrong, i thought if someone tell sthat he feels like he shud not live any more, it shud be understood that he is going for suicide very next.

but this proves me wrong thanks both of u, really helped me.

You have to ask if he has a plan, if there is medicine or a gun in his house... Sometimes the patient feels worthless, but not to the point of planning suicide. If the patient does indicate he thought about how he would do it, hospitalization is indicated. You may hospitalize against his will, but if he refuses to take medicine, you can't force it.

ECT is only for refractory cases of depression, you have to try other treatments first. But it's very effective and safe.
