Hi, Everyone is doing great over here! I have been very lazy after the result. Was busy with renewing passport etc. I have started with IM, gone through 25% of the book. But I need to concentrate alot more.

Hello journal....its evening here now. i will start studying now. yesterday returned at night and ws so tired. today i ws out till now. works done .. now i will try not to go out as much as possible.

hi MD, sorry, was busy in weekend,dint do anything but yesterday, got rid of infectious d's from IM, n today, time for paeds infe d's....where r u at???doing GIT together or wot???
few questions:
GIT:
>what do you do if you in x-ray find osteomas as an incidental findings??
>what would be the next step if culture report of endocarditis suggests infection with streptococcus bovis??
>How will you confirm your diagnosis of diverticulitis ?? and why??
>what are the antibodies associated with ulcerative colitis and crohn's d's??
Infectious D's:
>preferred antimicrobial agent active against anaerobes??
>what is the importance of respiratory rate in determining the severity of pneumonia, how??
>what are the differentiating features in physical examination between otitis externa and media??

heyy...as i told..i cudnt do in the wkends and ysterday as had to do some pending works.
today ws home..bt still lazying a bit.. still infc dis is not done....tried to finish today..lets see how far i can do in next hr. and noooo....not doing GIT or anything else...no no..cant go on like this..
lemme try to answer few qs..
1. osteomas..hmm..investigate for FAP ?? colonoscopy next step??
2.strep bovis....as far as i rem..in step 1 UW thr ws a qs....association with colon cancer ..so work out for that ??
3 n 4.have to read..
ID:
1.metronidazole .. under the diaphragm..and clindamycin for above..mainly for chest..
2.resp rate gvs approx idea abt consolidation...more the consolidation..more hypoxia..hyperventilation....more the resp rate. so it is a good indicator of severity. more than 20-25 is very severe.
3.hvnt yet read that here..bt do hv some knowledge ..bt will first read frm book and then answer.
now, i am going back to Inf dis.

hey nany..will try to do that.. cudnt report for last 2 days as net ws toooo slow !! and cudnt open the site for once. even now it is getting disconnected every 2 mins..so lemme post this now. well..last two days..i had to go out again....just tired of it.....and i guess this wkend..i will hv to go out again.

hi MD, hey, i read ur answers, good job dear......
answer of d rests, i m posting here....
1> ya, right, next step would be to do colonoscopy if u find an osteoma, it is called Gardner syndrom( association of colon cancer with osteoma)
2> u r right, there s associaton between step bovis endocarditis and colon cancer, so one must have to screen for that , so do colonoscopy.
3>for diagnosis of diverticulitis( or whenever u suspect any infection), always go for CT scan to confirm ,as barium study and endoscopy are contraindicated because of risk of perforation.
4>antibodies: ulcerative colitis-ANCA(antineutrophil cytoplasmic antibody)
crohn's d's-ASCA(anti-saccharomyces cerevisiae antibodies)
1,2 - u did gr8 job
3>otitis externa-pain with movement of tragus/pinna (not with otitis media)
otitis media- red,bulging tympanic membrane with loss of light reflex, and immobility of membrane with air

Ok....done with Infectious dis ....atlast !!
took too long....as cudnt study at a stretch....and also ws nt getting the rhythm....bt i guess we need to go on studying and we will gradually get it !!
cant do same inf dis from peds again.... ..will start with smthing diff.....Ok..lets do Obs part..on sunday i wont be home..lets see how much i can do tomorow !!
Hey hir, u finished peds IDs ?? know what....i thought i hv written Gardner syndrome in my answer....bt now i saw..i forgot to mention that !!
and abt otitis..i knew this..bt ws nt confident to write this expln..thinking smthing else might be there....this is wat cald lack of self confidence !!

hey, MD, so done with ID, gr8!!!!! and u finally jumped over to obs....ya, thats true , i also find IM very boring but just done with paeds ID.....now, i am thinking....wot to start????? should i go for surgery????????
again weekend arrived, i m sure, i wont be able to do anything....n there is also "superbowl" on sunday , so i think its pardonable
anyways, i hope u ll study well,,,,,have a gr8 weekend!!!!!!!

i ws doing peds Inf dis..just half way thru'....tomorrow wont be able to study much !!

hey hir..i ws sad today....so didnt post till now..thing is....i am getting distracted !! for some reason..so not putting that effort !! feeling so guilty..cudnt finish inf dis from peds....i dont want to continue like this....pls..we need to set some specific target for each day .. specific chapters !! otherwise....i will lose time again !!

hey MD, dont be upset!!! everything will be ok......
ok, tell me, hw u want us to do...if u want to set a time-table for each day, then its fine, we can do that....no problem
wot i m thinking is,,,,,,lets change the sub....we should start surgery, its interesting too....so let's say, we ll give just 4 days to surgery...urs-tue to fri, mine -mon to thur....there r 2 sections-2 days for each of them, then again devide 6 chap in each section....so we have to do 3 chap a day from kap notes......
wot do u think?????

Ok then.. lets start with general part...but first 6 chaps are small..arnd 50 pgs while next 6 almost 150 pg....so i guess we shud try to finish the first 6 chap in one day..and next 6 in 3 days.....wat do u think ??

oh ya...u'r right...i dint check that....ok then 1st day for 1st section...so our target would be 50 pages per day, right????...and 5th day for que from q book..thats it....5 days schedule for surgery....
we have to do this...n we ll do this.....no excuses from any of us.....wotever extra work than study comes , thats our personal problem, but we have to report our targeted work at the end of day to each other......u ll report at the end of day, n at the same time, i ll at d beginning of day about d previous day.......is that ok with u?????
ok then, lets start with that........till then no more time-pass in the forum.......
