A 62-year-old woman has been admitted several times in the past few months for fever workup. She lives on a cattle farm in the midwestern United States and has, until this past year, been in excellent health. She tells the team that she had an episode several months ago when she experienced muscle aches, night sweats, a profound headache, and cough. Her husband says that her eyeballs turned yellow. On this present admission, a new heart murmur is noted. Blood cultures for endocarditis have returned negative results. What is the organism most likely to be causing her fever? A. Coxiella burnetti B. Leishmania donovani C. Shigella flexneri D. Streptococcus pneumoniae E. Treponema pallidum
