aspirin toxicity mixed disorder
1 ph 7.36 pco2 22 hco3 12
2 ph 7.29 35 14
3 7.45 30 20
ans 1 please explain approach &ruling out others

Aspirin toxicity is explained very well in usmle world and kaplan notes. i personally do aspirin toxicity questions by ruling out wrong options...it takes longer but its like a double check.

aspirin toxicity has 2 components...initial hyperventilation due to respiratory centre stimulation and resulting respiratory alkalosis and second state of metabolic acidosis...so u have to look for the option with near about normal ph...coz its mix of alkalosis and acidosis. and u have to look for low pco2 due to hyperventilation and low bicarb due to acidosis...so the correct answer is 1.

2. has slightly low pco2 and low bicarb but the low pco2 here can be called compensatory. in aspirin toxicity the pco2 is lower than expected for compensation...

i will go for 1
aspirin toxicity causes both repiratory alkalosis an met acidosis so ph should be somewhere near normal
3 is not rt since in met acidosis ph should be low pco2 an hco3 are low but ph is not
2 is met acidosis
whats the answer?

if u have to consider near normal ph why not 3 ption
