female on birth control pills, now has increased LFTs, ascites, and hepatomegaly. next step? 1. doppler of hep vein 2. CT looking for adenoma

answer is doppler. (hepatic vein thrombosis)

why not ct it cld be an adenoma the q doesnt give any indication of symptoms of thrombosis?

Unlikely.....two main reasons....... 1....hepatic adenoma is rare but i guess for the exam ur index of supicion should go high when the patient is taking OCP or an athlete is taking Androgens.......but that is the exam 2....suddenly ur index of suspicion should go real low when u see the patient having ascites........ If hepatic adenoma is rare then trust me Hepatic adenoma with ascites is extremely rare...... ...things to look for when u get a case of suspected hepatic adenoma...are: 1....Female on OCP 2...Males on Androgens otherwise ...it might occur in patients with glycogen storage disease or hemochromatosis (but for the sake of the exam i am sure u wld never be asked that) 3.... Any of the above two with sudden development of what may appear to be spontaneous intra abdominal hemmorhage.... then go look for hepatic adenoma in the answer and ur test of choice wld be CT scan..... but if there is Hemorrhage and the patient is unstable then...ur next step wld be ...abdominal sonogram with DPL hope that clears it all this is all u need to know about Hepatic adenomas....... and anything that doesn't conform to this ..then it isn't it..... O yeah another thing....stop the OCP ...the adenoma might regress....and go for surgery.....
