A 59 year old man is post op and underwent a uncomplicated subtotal thyroidectomy. Shorty after the surgery while in the post-anesthesia care unit, the patient begins to complain of difficulty breathing. The patient's breathing becomes more labored, and he develops stridor. On physical exam, his neck is tense and some blood is seeping from the wound. Which of the following is the most appropriate initial step in management?

A Aspirate the hematoma using a large-bore needle
B. Intubate the patient iwth an endotracheal tube
C. Open the wound at bedside to evacuate the hematoma
D. Perform an emergent tracheostomy
E. Return to the operating room to control the bleeding


Also, please explain ALL the steps you would do in such a case.

I would go with B as the initial step. Then E.

That was my reasoning as well, because I thought an expanding hematoma in the neck means he will soon lose his airway and it's an indication for intubation... mad
but I guess that was wrong in this case

This is the right answer word for word:

The correct answer is C.
The patient's respiratory distress is the result of postoperative bleeding causing airway compromise. The very first step would be to open the wound at the bedside to adequately relieve the hematoma, which is much easier than using a large-bore needle to aspirate and thus creating another wound site (ChoiceA).

Once the hematoma is evacuated, the patient should return to the operating room (choice E) for control of bleeding.

It is difficult to intubate the patient (choice B) or to perform a hracheostomy (choice D) until the compression caused by the hematoma is resolved.

intubation has its complications think of much easier manuevre before intubating

I knew we should open the wound at some point, but thougt of intubation as a MUST...sad
