Why do we use heparin in pregnant patient who was diagnosed with antiphospholipid syndrome and has a platelet count 70,000? Thanks

Antiphospholipid syndrome is associated with increased risk of coronary heart diseae resulting from thrombosis. So I guess heparin would be a prophylactic measure for such patient. However, the low platelet count + anticoagulation may predispose to bleeding, and here comes the contradiction.

Anyone?

antiphospholipid syndrome..incresad risk of abortion from placental infarct..rx with low mol wt heparin...platelet count less than 30000 increase risk for bleeding

since APS is an autoimmune disorder characterized by hypercoagulable state with recurrent thrombosis that affects any organ system, so anticoagulation is the treatment.

for obstetric patients with APS, standard therapy is subcut. LMWH and low-dose aspirin. if she has recurrent history of fetal loss, then it is given prophylactically and withheld at time of delivery and then restarted after delivery upto 6-12 weeks postpartum. warfarin is contraindicated. these patients also receive calcium and vit. D supplements throughout pregnancy to prevent heparin-induced osteoporosis.

What about the low platelet count? Won't it induce a bigger chance of bleeding when combined with an anticoagulant?

that's right, i just thought that heparin might be contraindicated in patients with low platelet count, anyway thanks guys
