Seven months after the death of his father, a 27 year old graduate student with no prior psychiatric history experiences irritability, crying, difficulty in concentrating, and trouble falling asleep. When he's with his friends, he frequently talks about his father and how much he misses him. once a social person, he has been withdrawn and rarely goes outside his home. He feels excessive guilt, and wishes he had spent more time with his father prior to his death. outside of a 10 pound weight loss, there are no significant findings on physical exam. Which of the following is the most likely diagnosis? A. Dependent personality disorder B. Dysthymia C. Major depressive disorder D. Normal bereavement E. Posttraumatic stress disorder Please explain how we distinguish between Grief (normal bereavement) and Major Depression, ESPECIALLY BEFORE 1 YEAR since the death. Thanks.

I go with d I think that normal if the person says that he wish it was him and not the father. ABNORMAL when start having thoughts of killing himself

d complete feeling of worthlessness,suicidal ideations etc are more towards major depression

hey...Normal bereavement should NOT affect the patient's social functioning..Given that, he is isolating, ibereavement goes down the list. With respect to MDD, 5 symptoms of SIGECAPS in 2 weeks.....fits in well...Ans: MDD

With the pt's length of feeling the above symptoms, it's simply major depression. In order to distinguish most of the psych. dx. we have to look at the time since the pt. has been having probs. In this specific case it's more likely to have the DDX bet. Major depression & dysthymia.
