I have a question. Is that OK to have a little discrepancy between the history taking and PE, which is SP's part of the case and the PN?

Document whatever you find in the patient...do not make up stuff...so no it's not ok for there to be a discrepancy. The SP plays an important role in the case as he/she is the one to rate you on whatever happens in the room. The physician will have no role on what happens in the case. The PN is the opposite, the SP has no role and it will be graded by the physician. The physician knows all the positive/negative findings in the patient so don't write down something you don't find...
