well if you have some valid excuse like a death in the family or you got really sick or something like that you could say that....
But if not then just say you tried and failed, but you didn't let that get you down... this shows you are not a quitter and are very determined to accomplish your goal in life of becoming a doctor. I always say it's only considered failing if you quit and give up. As long as you get back up and try again, thats what counts.

Another thing: If you only failed by a couple of points and then took it again and passes with an awesome score, you could say something like "I'm glad I got to take it again to show what I'm capable of" or something like that, but don't get too cocky.
