1) A 40-year-old man states that he is feeling somewhat down and in conflict with his wife. As your history proceeds, you elicit that he has always been somewhat rigid; he is a conscientious hard worker at his employment. You note that he is dressed very neatly and that, while giving his personal history, he rambles and gives you all the details. The most likely personality type is 1) passive-aggressive. 2) narcissistic. 3) obsessive-compulsive. 4) dependent. 5) self-defeating (masochistic). Is it 1? or 3??? 2) A panic disorder with agoraphobia has been diagnosed in a 33-year-old patient. Which one of the following drugs is LEAST likely to be useful in preventing panic attacks? 1) clonazepam 2) paroxetine 3) alprazolam 4) buspirone 5) desipramine

Let me again: 2- 4 Buspirone often for chronic anxiety, not acute like panic.

this recalls me of another Q from UW, it asked about the peronality defect?? the scenario was that a lady calls up the physician in person and tells her that she is been trying for an apoointment past 2 months but everytime she calls up for the appointment the assistant lady says that the doctor is very busy these days. it all happened after she had a words with the asstt. lady of the doctor regarding some reports which were given very late... soon after that she never talked properly to her and never gave appointment to meet the doctor. options were many but the correct answer according to UW guys was, PASSIVE AGGRESSIVE pesonality. these Q are really confusing and i dont know what to do to avoid mistakes

The doctor's secretary is passive agressive because she doesn't like the patient, so she doesn't cooperate with the patient, but does not confront her directly, either. It's like when you ask your husband to buy Tampax, which he hates to do, he says he will, and then he does not. Passive agressive. Avoids confrontation. I think the guy in question 1 is obsessive-compulsive, poor wife! Narcissistic would feel entitled, better than other people, would demand privileges. Dependent would be clinging to somebody. Masochist would look for situations that would make him suffer. A joke: masochist tells sadistic: hit me! Hit me! sadistic answers: I won't! I won't!

Patient personality style Dependent (for my opinion is the only one could fit with this pt description) - has a need to be cared for by others, resulting in the desire for excessive attention from the physician. (came to doc complaning on (!) feeling somewhat down and in conflict with his wife (needs attention -1), hard worker - (needs is attention -2), gives you all the detail on his personal history(needs attention-3) Passive -agressive - asks for help but does not comply with the physician advice Obsessive - compulsive and Type A - - fears loss of control and may in turn become controlling diring ilness - characterized by time pressure and competitiveness - may also show hostility which is associated specifically with development of CAD Narcissistic - perfect self-image, feels superior, require "top" physician

why Obsessive-compulsive: Perfectionistic ( dressed very neatly, conscientious hard worker, gives all the details) Orderly Inflexible: "somewhat rigid" Indecisive

Why not dependent: Allows other people to make decisions and assume responsabilities for them(this patient is somewhat rigid, seems that don't understand others point of view) Poor self confidence(I don't think He is compensating trying to dress nicely)

ok, could be OCD. But chief complain as i understand - feeling somewhat down - ? What is that if not seeking attention after not getting enough from conflicting wife? Somewhat rigid, nicely dressed - could almost everybody fit i this category, why pt sees a doctor - that is the problem. Poor self-confidence AND fear of being deserted and alone - brought him to doctor all this is too hazy for me
