A 42 yr old alcoholic presents to the emergency department complaing of 12 hrs of epigastric pain radiating to the back. One and half hrs ago he began to experience nausea and vomitting. His temperature is 38.4 C, pulse is 100/min, blood pressure is 110/70 mm Hg, and respiration 18/min. Abdominal examination reveals mid-epigastric tenderness, without guarding, rebound tenderness or distension. Laboratory tests shows serum amylase of 1050 U/L, AST of 300U/L, WBC count of 18000/mm3. A plain film of abdomen shos several small bowel air fluid levels.
which of the following is most likely diagnosis.

A) Acute cholecystitis
B) Acute gastritis
C) Acute pancreatitis
D) Intestinal obstruction
E) Perforated peptic ulcer

Q seems easy and i feel most of u might have come to the diagnosis as well....but plz tell me what is reason for the air fluid level in the x ray.

C - acute pancreatitis.

ileus which ccurs in this case is due to irritation of bowel adjacent to the pancreas...this x -ray sign is called SENTINEL LOOP.
