kindly tell me what all clinical clerkships you are to write in form L5
does tht include only the hours u attended in internship.....?or also includes the clinical postings u attended during ur medical school in all subjects....

what dates will i write in clerkship period ...they ask for dates but in our transcript it is written just in form of hours.........

Either you should be having those dates, or your office in Med schoool will be having them, So ask your med schol office to fill out the form..................This is the only bottle neck for getting the PTAL.

Download PTAL form on their website, read carefully and DO EXACTLY AS THEIR INSTRUCTIONS.

i hav actually read the instructions n they say i reqire 8 weeks in surgery medicine 6 weeks in paeds 4 weeks in psychiatry n so on.....
i hav all of them except for the psychiatry part.....actually i dint hav my rotation during my internship in psychistry....so can i write the weeks of my psychiatry clinicals i attended in my mediacl school....n get their dates filled up 4rm them n then get tht form signed from dean....
so will CA peopl correlate again with my transcript........
