Hi I am dowjunk, cool24, graduated in Aug07, presently doing my internship in Pakistan, with a couple of research projects in line.

AIMS: undisclosed sticking out tongue

Step1 target triad: June, July August 08 (SD +/- 2motnhs) raised eyebrow

Starting: I've been on and off madthe stp1 track for the last many months, due to final year studies + research projects + home chores + chillin &cool hanginout e friends, lol! Ok my fresh begining comes up on Fifth January 2008.

Chalkin out my plan: Lets start with thewink 3Ps. Although during my past broken step1 spells, I did anatomy incldng nurophysio from Kaplan, 60% of patho from Goljan (both audio and book). But I wont count them in my new status; hope it helps in the journey.

My rolecool: I will keep updating on regular basis, will attach the best of thenod'stuff' that I know of and have grabbed on the way! I'll keep a check on myself and my friends who join in.....will keep an eye on other preps and will motivate others. I'll pinpoint unorthodox confusedstuff, will ask thrilling raised eyebrowquestions and creepy rolling eyesissues, will answer queries and questions, will pray for myself and for others.

& my favouritesticking out tonguepoint; Your role: You'll have to keep me going, as I keep you rocking! Put in your views, the tough stuff that you find in the books, the rough rugged instances that bother you, the issues that slow you down. Share your links, your work, your strengths and weaknesses. But please DONT shaking headBE BORING, or I'll tickle yousticking out tongue!



I know that its The Almighty Allah Who has The Power, as our weak souls can work the hardest, and pray to the utmost,,,,,,,,O' God help us in our journey, help us be the BEST! Please O' Lord make us the most successful in both the worlds!

January 2008: I'll start e Physio. Kaplan and BRS will be my pals for the time being. I'll do GIT and Kidney from BRS, others from Kaplan. If time permits, will do neurophysio and respi from both the books!

6 Jan 08: I m rocketing thru Chp2, BRS,Neurophysiology and GIT, from BRS! I intend to finish with it by the end of Sunday 6th Jan08.

Will discuss HOT SIZZLING POINTS of these 2 chps tomorrow hopefully.

8Jan08: UUUUUHuuuu I drained 500 ml of pleural fluid today, all on my own. Came late.

This neurophysio is cooking my head furthermore, so I'll finish it off later. Am starting Kidney from BRS, for now.

HOT SIZZLING ISSUES of GIT:
1.Gastrin secreting tumors cause hypertrophy + hyperplasia of stomach!
2.Gastrin is released in response to vagal stimulation, mediated by GRP(not acetylcholine).
3.For artificial insulin release give ORAL glucose. 4.Enkephalins are neurocrines, that inhibit intestinal secretion; can be used in diarrhea. 5.Intraesophageal pressure equals intrathoracic pressure; so ballon cathter helps!!! 6.Most muscles of esophagus are stimulated by acetylcholine (parasympathetic) during deglutition, except LES where acetylcholine is replaced by VIP (relaxer!!!) 7.Stomach empties fast with isotonic contents, while hyper/hypo slows is down! 8.Vagovagal reflexes incld gastroileac reflex, gastrocolic reflex, gastric receptive relaxation 9.Why vomiting causes metabolc alkalosis while diarrhea causes hypokalemia, predom. 10.Saliva is hypotonic e >K & HCO3, HCO3,

A day full of surprises, but disappointingly no time for books.....M tryin to eat up Kidney within the next few days, from BRS. Hopefully!!!

20Jan08 some long frustrating days, as I realise I have slowed down again. Reasons for this lazy syndrome: 2 days sick, 2 calls of 34hrs per week!

So I have finished kidney from BRS, presently trying to go over respiration from Kaplan (with a close look on BRS)!

HOT SIZZLING ISSUES OF KIDNEY: (acid-base will come up later)
  1. diarrhea is loss of isotonic fluid=ECF dec, ICF unchanged. Sweating is loss of hypotonic fluid =ECF shrinks resulting in ICF shrinkage.
  2. Renal Blood Flow (RBF) is 25% of carrdiac output.
  3. In old age, GFR decreases. Serum Creatinine is also dec as muscle is mass diminishes in the old. These 2 compensate each other.
  4. 20% of renal plasma is filtered across the glomerulus.
  5. When Glucose level is <250mg/dL, all filtered gl is reabsorbed. At levels > 350, carriers become saturated!
  6. Highest Clearance: PAH>K>inulin>urea>gl, Na, HCO3, aa: Lowest Clearance
  7. Thick ascending limb, EARLY distal tubule are IMpermeable to water.
  8. More anions in lumen OR raised intracellular K = Increased K secretion
  9. Loops dec body Ca, while thiazide increase it!
  10. Mg and Ca compete for absorption sites thru lumen in the ascending limb.
  11. CORTICOPAPILLARY OSMOTIC GRADIENT (formed by urea and NaCl) ranges from 300mOsm/L in Cortex.......while 1200mOsm/L in papilla. THANX TO ADH!
  12. The final urine osm is the same as that at the tip of U of loop of Henle.

Kidney's formulae:
  • Vol=Amt/Conc (used when vol of body fluid compartment has to be measured)
  • Clearance from Plasma= U(urine conc)F(urine volume/time)/P[siz e="1"]c [/size]
  • RPF = UF/P (PAH is filtered + secreted by renal tubules, therefore it is used to calc the Renal Plasma Flow. Hencel clearance of PAH=RPF)
  • To calculate RBF = RPF
  • GFR = UF/P (where Inulin is considered as its only filtered, neither secr nor reabsrbd
  • FF=GFR/RPF
  • Filtered Load of a substance = GFR multiply by Plasma conc of subs
  • Exc rate of a subs = Urine Flow Rate multiply by Urine conc of subs
  • Reabs Rate = FL [font size="5"]- ER[/font]
  • Secr Rate = ER [font size="5"]- FL[/font]

I am done with physio, and will start pharma tomorrow isA. Some pals asked me to cut short nodwith the hot sizzling issues; so I'll take their words.

Hello party grin ... i should kick my past out of my mind. i was going thru physio on/off along with DVD lectures. but now m totally pissed off. m starting with endo. i have done 2 chaps today n will be heading for more after this post. n e ways. this is the begining we will be in contact now n on. raised eyebrow

Yeh i was going through the figures of cortisol and aldosterone... m bet sure putting my brain n just making concept wont b enuff for me so drawing huge pics in my brain of those figures ... ofcourse on my writting pad as well nod
but m bet sure first read must have good concepts wat so ever it takes... m giving more time on one single topic rather then just passing it by reading it like a flashing car. so hopes for good result in the end of this endocrino.. part. cheers. wink

Except for the genetics potion in general patho, I have gone thru the stuff from Goljan.

I 'll take along Special Patho and Biochem, to make sure that I pick up the marathon study pace! and do not get bored due to monotonous pattern! Both things are very crucial for me: Patho as it should be, and as it is for every1,,,,,,,,,,,,,biochem because I am very very weak in the molecular portion as well as genetics.

Lets keep rocking!
