A 21 yo M with no significant PMH presents to your office with blood in his urine and mucosal bleeding while washing his teeth. The patient c/o intermittent "ringing in his ears". He denies any drug or alcohol abuse. He has no family history of bleeding disorders. Petechia are noted in the oral cavitie, as is dried blood in his nostrils. Labs: hematocrit: 32%, WBC: 8000 with 60% neutrophils, platelet count 13 000, PT: 13 seconds, PTT: 28 seconds, LDH: 1200U/L, elevated indirect bilirubin. Coombs' test is positive, abdominal examination is unremarkable and spherocytes in peripheral smear. What is the most likely diagnosis: a. Alports' syndrome b. Bernard-Soulier syndrome c. Feltys' syndrome d. Thrombotic thrombocytopenic purpura e. Evans' syndrome f. Idiophatic thrombocytopenic purpura Explain your answer pz

A...tinnitus makes me think of it..also being a collagen disorder it causes mucosal bleeding??

EVANS SYNDROME---Autoantibodies are directed against specific antigens specific to red cells, platelets, or neutrophils, but they do not cross react Evans syndrome is a diagnosis of exclusion Signs of thrombocytopenia include purpura, petechiae, and ecchymoses. Signs of anemia include pallor, fatigue, and light-headedness. Jaundice may indicate hemolysis

Yeah Evan's is ITP + AIH but I think I ll go w/ A, Alports bec of the pt's age, blood in urine & hearing prob.
