3. , developing of antibiotics, I don´t think that people in Africa(most of third world) rapidly increased general living standards. The big factor of decreasing mortality was prevent the mortality of new born child(what was one of the biggest problems), than using antibiotics to threat variola....
I think antibiotics

Global (devloped + devloping) decline is for Antibiotic (especially PCN after WW2), but the question is asking about devloped country; it should be living condition.

4- I have no evidence but 4 is reasonable. In addition, Do remmember that after World War II, the Baby Boomer generation, especially in the Sates and Western countries, made mortality "pseudo-decrease" and a plus for sure is improvements in general living standards as well.

to decrease mortility you have to increase treatment, so the only option is C
