The following laboratory values were obtained during investigation of a 45-year-old man:
Hemoglobin
90 g/L
Mean corpuscular volume 
82 fL
Mean corpuscular hemoglobin concentration
300 g/L (N = 310-360 g/L)
Serum iron
7 µmol/L (N = 13-27 µmol/L)
Total iron binding capacity 
36 µmol/L (N = 54-72 µmol/L)
Sideroblasts in the bone marrow
0.25 (N = 0.30-0.50)
Bone marrow iron
Normal
These results could be due to all the following conditions, EXCEPT


    1)    iron deficiency anemia.

    2)    alcoholism.

    3)    chronic infection.

    4)    malignant disease.

    5)    rheumatoid arthritis.

A yes the TIBC should be increased
I will say that all other options can be under anemia of chronic disease

that's right but in alcoholism mcv is high right? I think it can't be alcoholism either.
confused between 1 & 2?confused

alcoholism can cause sideroblastic anemia which is small MCV
so it can be both large or small
