the Cardiac reserve will be decreased cos the LV is pumping more blood although the child may be at rest but the heart (espLV ) is working overtime to pump oxygenated blood. As half of the blood gets pumped back into the PDA to the Pulmonary vasculature choices D and E get ruled out. Now Choice C is the myocardial blood flow which should be decreased in under resting conditions but the LV is doing extra amount of work so therefore flow would be increased. Choice B is not correct for obvious reasons. The only thing which actually strikes the mind is the Choice C where one would assume normal flow but i dont think it would be normal. Will get back to with some reference as i m sure i read this somewhere.

Actually pulmonary vascular resistance (choice E) is also decreased as the explanation itself states. As the question is "What is most likely decreased ?" the better choice will be cardiac reserve.

first of ALL: cardiac reserve should be defined... cardiac reserve means in lay man terms the ABILITY OF HEART TO INCREASE CARDIAC OUTPUT ABOVE ITS RESTING LEVEL... in normal people like us...we can increase our cardiac output upto 4 or 5 times..from resting level of 5 litres/min to 20-25 litres/min... whenever there is CARDIAC FAILURE... I am makin a generalized term here... especially the SYSTOLIC failure as in this patient where the left ventricle is already dilated... the ejection fraction and cardiac reserve fall down... so its pretty simple question if you know what CARDIAC RESERVE means...
