You can only extend your eligibility period once for an extra 3 months. You can't extend it twice, unless they changed the rules recently. You will just have to reapply and pay the fee again if you decide not to give it. By the way, you said it expires in 5 days, shouldn't it expire at the end of the month on the 31st? Or do you mean 5 business days?
