A 10-year-old male presents to the pediatrician because of problems while eating. He can chew food but has problems swallowing the chewed pieces, and feels them in his throat. A radiographic study is performed that shows normal musculature but abnormal esophageal peristalsis. No masses are seen.

Which of the following nerves should be tested for abnormalities?

Upper two-thirds of the esophagus is supplied by the vagus nerve and lower one-third is supplied by the splanchnic nerve.

Abiltiy to chew but the unability to swallow and abnormal peristalsis guys connect the dots. This was a simple one but really had to know that action of vagus. Glossopharyngeal nerve has only one motor content which is to innervate the Stylopharynegeal muscle which helps in the Gag Reflex, otherwise its mainly sensory.

The correct answer is E.

The upper 2/3 of the esophagus contains striated muscle. It is derived from the pharyngeal arches and innervated by the vagus nerve (CN X). The lower 1/3 contains smooth muscle from splanchnic mesoderm and is innervated by the splanchnic plexus.

The hypoglossal nerve (choices A and B), or CN XII, moves the tongue.

The phrenic nerve (choices A and D), derived from C3, C4, and C5, innervates the muscle of the diaphragm.

The glossopharyngeal nerve (choice C), or CN IX, functions in taste, swallowing, and salivation, as well as monitoring the activity of the carotid body.
