A 21-year-old male presents with three days of abdominal pain and tenderness in midgastric and lower abdomen. He was febrile (39.2oC), tachycardic and tachypneic. He was admitted and blood cultures were sent to the laboratory. On an exploratory laparotomy, he was found to have a gangrenous and perforated appendix.

The blood cultures were positive for a Gram neg rod. What is the most likely organism?

A Bacteriodes fragilis
B Bacillus subtilis
C Bacillus cereus
D Clostridium perfringens

i think perfringens give a toxic picture n secondly its gangrenous!!!so i will go wth D, r u sure wth the answer satheesh? can u plz elaborate ur justification peekay?

BACTEROIDS ARE NORMAL FLORA OF G. I TRACT.AND IF I REMEMBER CORRECTLY IN APPENDICITS THEY ARE FAR MORE COMMON.

clostridium is gram positive, but you are right that it may be a good ans. i the q they asked the gram negative rod, so Bacteriodes fragilis is correct

oh yes ,i missed gram negative but if tht wouldnt have been in the ques i would have checked perfringens. :lol:

yes bacteroides fragilis...mode of spread via bowel defects(here perforated appendix)
