A patient is diagnosed with follicular lymphoma. Karyotypic analysis reveals a chromosomal translocation,t (14;18), involving the immunoglobulin heavy chain gene. The most likely gene affected during the neoplastic process:

a.-p53

b.- c-myc

c.-bcl 2

d.-bcl 1

e.- N- myc.



Actually ,the correct answer is c- myc. But i don t really understand why. May someone explain this to me.

To my humble knowledge from step 1 , c-myc is associated with Burkitt lymphoma (t 8 ,14 ) Now , this dude has follicular lymphoma (t 14,18 ) so my answer would be bcl-2 but I remember reading in NEJM that follicular lymphoma can have c-myc and bcl-2 working synergystically specially in follicular lymphoma grade 3 b where the is a deletion of c-myc into the immunoglobulin heavy chain .

I think that it is a very tricky question because most people would jump to follicular lymphoma associted with bcl-2 , but since the translocation involves the immunoglobulin heavy chain , they answer would probaly be c-myc

That s my 2 cents .

I will go for bcl-2. Immunoglobulin heavy chain on chromosome 14, bcl-2 on chromosome 18 and c-myc on chromosome 8. So it said t(14:18) and should be bcl-2 rather than c-myc. Please post the full explantation. thanks.

I'd have gone for bcl-2 too! Same reason as drgho gave! Waiting for an explanation for c-myc!

drgho,here is the whole explanation from "Robbins Review of Pathology" question book.This is just repetition of what doc_clotaire explained in a very clear manner:

"This is an example of chromosal translocation that brings a protooncogene (c-myc) close to another gene (immunoglobin heavy chain gene). The c myc gene comes subject to continuous stimulation by the adjacent anhancer ellement of the immunoglobin gene, leading to c-myc overexpression."

Thanks for sharing. but i feel the question and answer not match 'cause c-myc gene is on 8. hard to understand.
