1) in the step 3 form "date passed" for step 1, 2ck and 2cs is the date of declaration of result or date of the exam ?
2) what should one fill in the NID column if there is no national ID as such in one's home country ?

Put your PP number.That can be national ID if you don't have one
You don't need any specific date. Just write the month you sat for the exam
