Agree with neuro, tamerbashir, hopefor, guangyu. Qn 1) A. There is nothing to suggest Cushing's, per se. I know you can get central obesity in Cushing's, but in this case, it's more likely the young 15 yo has essential HTN, and obesity is a contributing factor. B. Insulinoma D. This looks like tension pneumothorax, pushing trachea contralaterally. Someone said that haemothorax does not result in hypovolaemic shock, which is not true. Bleeding into the thorax is one of the five potential sites of massive bleeding in trauma (the others are abdo, pelvis, external, long bones). Massive bleeding causes hypovolaemic shock.
