if an IMG gets a green card during the residency while he is on J-1 Visa, can he switch to permanent resident and therefore does not have to go back to his home country or work in an underserved area after residency???

not possible, you would have to wait till your J1 period ends to change your migratory status.

yea but what i was asking about is will i still have to go back to my country and wait for 2 years??

Yes, or get a waiver and go to an inderserved area. You don't escape J1 even if you'll have a green card in the future.

In fact you will not get green card till you fulfill requirements of J1 212 (e): 2 year home country or serve deprived area..
