A 55-year-old white male is brought to the emergency department for weight loss. He appears thin, emaciated, and unkempt. His accompanying social worker states that the man has not been seen at the local soup kitchen in the last 10 days. The patient demonstrates normal cognitive ability, no hepatosplenomegaly, and no fruity odor to his breath. Laboratory results reveal normal liver enzymes, insulin levels, and adrenal axis. The only problem you can discern is extreme, prolonged fasting. Which finding would be expected?

A. Blood glucose of less than 40 mg/dL

B. The brain uses glucose exclusively for energy demands

C. Decreased hepatic glycerol utilization.

D. Lower than expected hepatic gluconeogenesis

E. Low ketone bodies in serum due to muscle utilization

Damn i went the wrong way. Brain does use ketone bodies for metabolism thereby decreasing the needs for hepatic gluconeogenesis thereby sparing glucose for RBCs and spares muscle proteins from metabolism.

Thanks JR
