Does anybody know what are the documents required for a H1 visa application?

Step I,II, III results; medical school diploma, appoitment/contract letter, ECFMG cert. (not necessary)... things may vary person to person--- depending on preceeding stay/visa.

Normally, programs do not pay for the visa application right?

We will have to pay for it ourselves?

Add to that list - medical license for the state that you are going to work in.
