i interviewed at 3 programs that i really liked in october. just realized i forgot to send thank you to them. have i messed up with them?

No, the majority of applicants never send me anything. Send them now, it's not a big deal.

if i interviewed on october 5th, isn't it too late? also, do ypu remember who sent you them when you rank? also, bbb, you do a great job on this forum. if i want 2nd look at these places where i have not communicated after interview, what do i do?

No, it's not too late - again, most applicants don't ever send anything. It's also not to late to do a second look - include that in your thank you email.

bbb, is it awkward or weird for applicants to actually show intense interest in the program and ask for a prematch? If I am very interested in a program would it be appropriate to ask if they give prematches? Would they see that as flattering or would they see that as being to eager and agressive?

You will never know if a program offers a prematch unless you ask if they offer one.
