how to deferentiate 5 alpha reductase defeciency from testicular feminization syndrme based on physical findings????

plz reply soon... exam is on 29th

HI Mikky,

This is the way I see it: since you will have an increased testosterone/DHT ratio in 5ARD you will expect to see increased body hair like in pubic and axillary regions, increase in the prominence of Addam's Apple, increased muscle mass, etc. all of this is due to the increased levels of testosterone.

in feminization testicular syndrome, you will expect to see a normal testosterone/DHT ratio because the problem is not 5ARD, it's the androgen receptor insensitivity. So you will see the contrary, decreased of body hair, almost no breast formation since testosterone is not available for aromatization.

it's a little bit more complicated than that, but I remember I had this type of question in one of my Qbank, (probably UW).

Testicular feminization loooks like a female externally with well developed breast.

5 alpha reductaase deficiency leads to reduced amounts of DHT formed. This leads to poor development of penis and prostate. In testicular feminization there is resistance to the androgen sensitivity by the tissues, and the pt has female external genitalia with undescended testes. Correct me if i am wrong.
