34: (change my answer) it's exudative with lymphocyte predom. so it's either TB or malignancy. hx. of smoking points to malignancy. Hilar fullness would point to TB but that's not an opbion. Straw colored and transudate would be CHF.

39 is cardiac tamponade , cardiac output would be decreased

I think 21 is diagnostic bias

b is less invasive for beginning and ill show cardiomyopathy
