a 76 yr old woman was hospitalised for dyspnea an lower ext edema
she says that its very difficult for her to move around without exp fatigue.has a h/o chronic htn.on phy exam jvp is 10 cm above sternal notch,insp crackles at lung base and 4+lower ext edema.which of the foll would confirm diag
1 xray chest
2 echocardiography
3 endomyocardial biopsy
4 pulm func test
5 electrocardiogram
6 cardiac angiography

ANGIOGRAPHY FOR CORONARY ARTERY DS... to see any emboli or athero thrombus in vessels.
not for CHF due to heart valvular ds or cardiomyopathy.so. echo is best to see valves and cardiomyopathy.

2- Echocardiography defines EF (Systolic function), even defines diastolic CHF. Angiography can count EF, but just for referenc, not a major option, important for severe CAD. This case said that Echo is more cost-effective.

I also thought it would be chest xray first, and then read that initial test of choice is Echo when CHF is suspected as in this pt, based on history, examination or cxr.

Is there an answer for this smalley?
