67 yo male 3 day hx of fever and HA. aortic valve replaced 5 years ago for Rheumatic fever. Now appears ill with fever of 104 bp 110/65, pulse 110 and resp 22. systolic ejection murmur heard. left hemiparaesis, babinski present on left. no nuchal rigidity. Increased risk of which complication?
a. brain abscess,b. carotid artery occlusion, c. encephilitis, d. hydrocephulus, e. venous sinus thrombosis
Previously healthy 16 yo boy presents with fever and cough with right sided chest pain for 2 weeks. six months aog he visited his grandparents in albania for 2 weeks. 120 lbs and 68 in tall. he appears thin and pale. temp is 100.8 pulse 76, and resp. 36. exam shows shallow respirations with decreased breath sounds at right base. xray shows right pleural effusion and hilar adenopathy.
antitrypsin def, CF, pneumothorax, pulm. alveolar proteinosis, aspergillosis, pulm. hemorrhage, TB
67 yo woman comes in for routine health maintenance exam. vitals are normal. on labs she has high calcium ( 11.9) and low phosphorus (2.8). albumin 4.2, ionized calcium is 5.8 (elev). Next step in management?
measure alk. phos, measure PTH, measure vit. D, serum and urine electrophoresis, bone scan, x-ray, skeletal survey
77 yo female with back pain for 3 months. no pain on palpation, no costovertebral pain. reflexes are normal, no babinski. straight leg test is positive for pain at l2-l4. Treatment?
tylenol, gold, methotrexate, prednisone, probenicid
3 yo boy just completed 10 day course of amoxicllin for right ear pain. still has temp. of `102 with lateral displacement of right auricle and pain on palpation of post. auricular area. Next step for diagnosis?
bone scan, ct head, tympanomemetry, LP, tympanocentesis
