thank you angel, focus on your exam now, i think i will go for step 1 first, because i only watched internal medicine videos for step 2. so, long way to go, questions and all. but i dont know how to study for step 1 now, if i have done those questions, what will help me? anyway, i have to figure it out. angel, again, dont get distracted, and dont answer me now, your time is very important right now, so stay focused, dont get distracted by any other stuff. you have done really well so far, you will ace the exam, go for it!

kalyan, very nice words, yes, i have been thinking, i am fool, and inferior, almost everything you said, except for God, when i was younger i use to think that way, now, i dont get mad at God, i get mad at myself. i like the idea, God has better idea. but i feel old now, i feel i am taking too much time to start my career. ok, ignore that.

if not ,you can start with that , doing 100 q/day random mixed ,,
and begin with ur weakest subject from kln , couple it with first aid , and subjectwise qbook and and uworld revision ..
you know best where u went wrong /fell short
so work on that now

Since iron deficiency anemia doesn’t develop over night, some cells will be small, and some will still normal. This could be detected by high RDW.
Iron is usually stored in MacroPhage in the BM. In inflammation (chr. disease), our body responds as if it’s an infection ! we don’t want bacteria reaching our iron (because it increases their reproduction), so we keep it away from the bacteria, but also keep it away from the RBC. Serum iron!, even though we have a lot of stored iron ! not enough to make heme. TIBC decreases, ferittin increases.
We do not treat the mild thalassemias (alpha and beta). Don’t give iron !
Iron deficiency reasons: prematurity (give iron supplements), bleeding Meckel’s diverticulum in a newborn. Women: under 50: mainly menorrhagia. Under 20: anovulatory cycles. 20-40: ovualotry cycles with irregular shedding, inadequate luteal phase, pregnancy related bleed, endometrial polyp bleed. Men: peptic (duodenal) ulcer disease Over 50: colon cancer.
undigested iron tablets in a baby (mom with menorrhagia) can be seen on X-ray.
B12/folate deficiency ! dTMP deficiency ! immature nucleas ! big cells.
Purpose of cobalamine: taking off the methyl group of methyl-THF ! methylcobalamine ! gives the methyl group to homocystein ! methionine (needed for 1C transfer reactions). - Folate (less B12) def. ! increased homocystein.
"homocystein ! produce thrombosis including MI.
