Dexrazoxane is an iron-chelating agent approved to reduce toxicity; however, it increases the risk of severe myelosuppression. << Emedicine. ========================= Despite its promising protective cardiovascular effects, clinical use of dexrazoxane has been seriously questioned because of the associated severe myelosuppression, which is actually potentiated by doxorubicin. The possibility that dexrazoxane may interfere with cancer therapy has also been raised. In addition, it is not yet known whether any protection that dexrazoxane provides against early doxorubicin-induced cardiotoxicity also extends to late cardiovascular effects. Additional clinical trials are required to address these points. Treatment If the heart has been damaged by doxorubicin, treatment options are few. Typically, doxorubicin-induced cardiomyopathy and heart failure are refractory to conventional therapy. Abatement of clinical symptoms by the administration of digoxin has been shown to be only temporary. Beta-blockers have been reported to be useful in the treatment of dilated cardiomyopathies, including doxorubicin-induced cardiomyopathy. Some favorable results have recently been described in children who, in addition to traditional treatment for heart failure, received the 1-receptor blocker metoprolol. Within two months after metoprolol therapy had been initiated, patients' symptoms diminished, as did the echocardiographic and angiocardiographic abnormalities. The promising results of pharmacologic treatment of doxorubicin-induced cardiomyopathy need to be examined further, in well-designed, larger clinical trials. Currently, the only treatment option that has been recognized as successful is heart transplantation. <<< NEJM
