Here is my way:
Knock on the door, but do not wait for reply (not needed), enter
"Good morning/afternoon, Mr/Mrs XXX, I am ZZZ, an attending physician/pediatrician here... How can I help you??? "
Baseline: do not do it very long, the greeting should take 15-20 secs...
