the causes of both syndromes can be myriad like Lumbar stenosis (multilevel) Spinal trauma including fractures Herniated nucleus pulposus (cause of 2-6% of cases of cauda equina syndrome) Neoplasm, including metastases, astrocytoma, neurofibroma, and meningioma: Twenty percent of all spinal tumors affect this area. Spinal infection/abscess, such as tuberculosis, herpes simplex virus, meningitis, meningovascular syphilis, cytomegalovirus, or schistosomiasis Idiopathic, eg, spinal anesthesia: These syndromes may occur as complications of the procedure or of the anesthetic agent (eg, hyperbaric lidocaine, tetracaine). Spina bifida and tethered cord syndrome the difference usuallly lies in following: onset conus medulary synd is quick n bilateral cauda equina syndrome (CE) is unilateral and gradual CM has knee reflex preserved ankle lost CE has both lost CM sensations are usually lost around the anal region...consider that the last Sacral and coccygeal segments are lost.. in CE ...saddle anesthesia is present CM has more back pain CE has les back pain CM has frequent impotence and sphincter dysfunction that appears early in the disease... CE has late onset of these... causes are similar in both cases as told above
